Privacy Notice
I am committed to protecting the privacy and confidentiality of everyone whose personal information I process. This includes adults with learning disabilities who use my services, as well as family members, carers, paid support staff, advocates and professionals involved in supporting them.
Where this notice refers to "you", this may mean the person receiving psychological support or another individual whose personal information I process in connection with providing that support.
What information I collect
Depending on the nature of the work, I may collect and process the following information. This includes but is not limited to:

Personal details, including name, date of birth, address, telephone number and email address.

Relevant contact details, such as GP information, next of kin, emergency contacts, commissioners or care providers.

Information about your health, wellbeing, psychological needs, behaviour, support needs and any assessments or interventions provided.

Information shared by family members, carers, support staff or other professionals involved in your care.

Clinical records and session notes, which contain information necessary to provide a safe, effective and evidence-based psychological service.

Appointment, correspondence and billing information.
I only collect information that is relevant to providing my services.
How your information is used
Your information is used to:

provide psychological assessment, formulation, intervention and consultation;

communicate with you and, where appropriate, those involved in your care or support;

liaise with other professionals where this has been agreed or is otherwise appropriate;
-
maintain accurate clinical records;
-
safeguard you or others where necessary; and
-
meet my legal, professional and regulatory obligations.
Sharing your information
Your personal information will be treated as confidential and will only be shared where:

you have given your agreement;
-
it is necessary for your care or support;
-
there are concerns about the safety of you or another person;
-
I am required to do so by law or court order
Where possible, I will discuss information sharing with you before doing so unless this would increase risk or is not legally appropriate.
How your information is stored
Your information is stored securely to protect it from unauthorised access, loss or misuse.
I retain clinical records for 7 years following the end of my involvement, unless I am required to retain them for longer by law, professional guidance or insurance requirements. Records are securely destroyed once they are no longer required.
Your data protection rights
Under UK data protection law, you have rights which may include:

the right to access your personal information;

the right to request correction of inaccurate information;

the right to request erasure in certain circumstances;

the right to request restriction of processing in certain circumstances;

the right to object to processing where applicable;

the right to data portability where applicable.
Some of these rights may be limited where information forms part of a healthcare record or where I have a legal or professional obligation to retain it.
You will not normally be charged for exercising your rights. I will respond to requests within one month where applicable.
